Fixed an issue where graves saved the highest XP level a player had, allowing XP to be regained even when dying with 0 XP. XP is now handled correctly per death.

We have significantly improved grave placement in difficult dimensions like the Nether and End, preventing graves from being lost in lava or the void. Additionally, guardian spirits are now invulnerable to player damage, and other minor bugs, such as the double message on grave creation, have been resolved.
